The survey is short (about 10 minutes) and completely anonymous. We collect both numbers and stories to help us plan for the year ahead — and every response makes a difference.
If you choose to enter the draw for the Wellbeing Pack (by providing your email address in the last question), your data will only be used to communicate with you if you win. The draw will be taken on 14th October 2025. We'll only reach out to those who win.

1. Eligibility
Entry is open to residents of New Zealand aged 16 years or over.
Only one entry per person is permitted.
2. How to Enter
To enter the prize draw, participants must complete the ANXIETY NZ 2025 SURVEY in full.
Entries must be received between 17 September 2025 and 12 October 2025 (“Promotion Period”).
3. Prize Details
There are two (2) prize packs to be won.
Each prize pack includes: Wellbeing Workbook, Keep Card, Prezzy Card $25 – total value of $49.
Prizes are not transferable, exchangeable, or redeemable for cash.
4. Draw and Winner Notification
The winners will be drawn at random from all eligible entries on 14h October 2025.
Winners will be notified via email within 7 days of the draw.
If a winner cannot be contacted within 14 days, a new winner may be drawn.
5. Privacy
Email addresses submitted in the survey will be used solely for the purpose of administering the prize draw and contacting winners.
All data will be handled in accordance with the Privacy Act 2020.
Participants may request access to or correction of their personal information by contacting reception@anxiety.org.nz
6. General
By entering, participants agree to be bound by these terms and conditions.
Anxiety New Zealand Trust reserves the right to:
Disqualify any entry that breaches these terms.
Cancel or amend the promotion if necessary due to unforeseen circumstances.
The decision Anxiety New Zealand Trust is final and no correspondence will be entered into.
